Terms and Conditions of Healing Therapy Monthly Membership (as seen on membership form):

 

"By signing below, I authorize Healing Therapy Spa and Sports to charge a monthly fee of $89.99 on my debit/credit card on the (circle one) 1st 2nd 3rd 4th 5th 6th 7th of each month. 

 

By initialing, I understand and agree that:

______ Payments are recurring monthly and can only be canceled by me on or after the length of contract has been reached. Membership payments will continue after contract length, until cancelled by me.

______ Healing Therapy Spa may cancel my membership in accordance with the terms and conditions of this agreement. 

______ Healing Therapy Spa will charge me a cancellation fee of $50.00 to terminate my contract prior to reaching the length of contract and all credits accrued will be voided.

______ No refunds. All prices, policies, and services are subject to change at any time without notice.

______ I authorize Healing Therapy Spa to use my likeness and image for social media promotional purposes.

______I understand and accept Healing Therapy Spa operates in a safe and professional manner, however in the case of injury, Healing Therapy, its employees, or affiliates are not responsible and I waive my right to legal action."

Membership And Appointment Policies

24 Hour Cancellation/Rescheduling Policy

• Cancellations and rescheduling must be made at least 24 hours before your scheduled appointment/s. NO EXCEPTIONS.

• Any cancellations/rescheduling made within 24 hours will result in a loss of credit/s, and/or a cancellation fee equal to 100% of the service/s you have reserved.

Late Policy

• If you are running late, please provide a notice, otherwise you are considered a “no show” and you will have to be rescheduled to a different time or day at your cost.

• If you are running late, and have given proper notice, you will be given the remaining time for your scheduled appointment. Due to a high volume of bookings we are unable to accommodate late appointments.

• NOTE: Clients arriving more than 15 minutes late to their scheduled appointment, with no notice, will be considered “no shows” and will be charged as a Same Day Cancellation/Reschedule

 

Past Due Payment Policy

• If your payment cannot be processed at the agreed-upon date of each month, you will lose the credit for that month. 

• Failure to process payment for 3 months will result in a loss of any and all remaining credits and all membership benefits.

• NOTE: You are responsible for providing new card information if you cancel, lose, or replace an expired card.
